New Health offers two easy ways to complete your new patient paperwork online: by using the free cliexa-SENSE app which sends your results directly into your provider's chart, or by using standard web forms. If you have a smartphone or tablet, please choose the corresponding option below for instructions on downloading and using the cliexa-SENSE app to complete your forms. Otherwise, click "I do not have a smartphone or tablet."

Completing your patient forms online using the free cliexa-SENSE app.

  • Download cliexa-SENSE

    From your iPhone or Android phone, go to the app store and download the cliexa-SENSE app. You can also use the links below from your smartphone or tablet to go directly to the cliexa-SENSE page on your app store.

  • Check Your Email

    You should receive an email from [email protected] containing your password for the cliexa-SENSE app.

  • Log in to cliexa-SENSE

    Use your email address and the password you received to log in to the cliexa-SENSE app. It will then ask you to create a new password. Please keep your username and password in a safe place so only you have access!

  • Complete the Assessment

    Complete the screener questions, consent forms and assessments assigned to you.

Using to connect with your provider.

  • Check Compatibility

    New Health uses the platform for televisits. supports iPhone, iPad, Android, and any desktop or laptop computer using the Google Chrome, Safari or Mozilla Firefox web browsers. does not support Internet Explorer. Whatever device you use, please make sure it has a working camera and microphone!

  • Select Your Provider

    From the dropdown menu below, select the provider you are here to see and then click Proceed to Waiting Room.

  • Allow Webcam and Microphone Access

    After you click "Proceed to Waiting Room", the portal will ask you to enter your name and then for permission to use your device's camera and microphone. Please click allow when it prompts you; we cannot start a video call if you do not give these permissions.

  • Wait for Your TeleVisit to Start

    That's it! Once it's your appointment time, a member of our staff will initiate a video call with you to check you in, and then your provider will start your appointment.
